Michigan Tech would like to clear up confusion regarding visitors returning to campus on Monday.
Visitors must fill out the COVID-19 Visitor Tracking Form, before arriving on campus.
The form will ask your name, email address, phone number and reason for the visit.
This is required if you intend to visit the library, mineral museum, golf course, bookstore, or other retail outlets.
This also applies to offices and classrooms and will also cover the ice arena, when the time comes.
When you complete the form, you’ll receive an email with a confirmation number.
It’s recommended you keep the confirmation number on you, although the university will not ask for it at this time.
If you answer questions on the form, indicating you may be at risk for COVID-19, you’ll be asked to stay home.
Protocols for vendors and contractors remain in place.
