Portage Health Foundation 2019 Community Recreational Facility RFP

[Hancock, MI] The Portage Health Foundation (PHF) is releasing its second of three Community Recreational Facilities and Resources request for proposal (RFP). The intent of these targeted RFPs is to support projects that will have an impact on community health and wellness while addressing the systemic health needs and risk factors of the broader community. Further, it is hoped and desired that proposals seeking funding will have identified key components to support sustainability and/or systems change to ensure ongoing delivery of services to meet community need.

These targeted RFPs are a result of community conversations that express needs that include repairs, expansions, and upgrades to existing recreational facilities throughout the four-county community. These facilities are important to overall community health and wellness. The PHF Board of Directors moved to support three years of targeted granting specifically for supporting these recreational facilities and their resources. This call for proposals is specifically addressing PHF’s Community Health pillar. Community Health grants provide support for activities or opportunities that promote healthy lifestyles which improve the overall health of a community.

PHF will be accepting Letters of Inquiry (LOI) until February 1, 2019 at 3:30 ET. Although LOI’s are not required they are highly recommended by PHF. Completed grant applications are due to the PHF office by March 18, 2019 at 3:30 ET. Please note, PHF will not accept any late applications.
